Special Events Alcohol Request Form

**Alcohol  requests must be processed a minimum of 2 weeks prior to the date of the event.

**Student organizations are subject to the guidelines given in the Student Organization Alcohol Policy.

Alcohol Dispensing Information

What beverages will be allowed?

In what quantity will beverages be presented?

During what hours will beverages be allowed?

Number of bars or areas at which alcohol will be dispensed from?

Is alcohol consumption confined to this (these) area(s)? 


If no, describe.

What non-alcoholic beverages will be served?

What type of food will be served?

What procedure will be used to assure that only those of legal age will be allowed to drink alcoholic beverages?

I understand this form must be submitted at least two weeks prior to the event.

Note: The serving of alcohol will end one hour before the end of the event or by 12:00 a.m., which ever comes first. Identification bands may be issued to adults of legal drinking age. Any event where alcohol is served can be ended prior to the agreed upon time if USC Upstate police or special events staff deem it prudent.
