Policies

Please refer to the following policies regarding the use of these items and sign below:

1. This form must be submitted to the Office of Student Life no later than 48 hours before the date needed by. 
2. Tents and PA Systems must not be used in inclement weather (high winds, hurricanes, tornadoes, heavy rain, etc.) 
3. Tents and PA Systems must not be left out overnight. 
4. More than one person should be available when picking up the tents, as they are awkward and heavy. 
5. In certain situations, the Office of Student Life may require a tent to be used over the PA system. 
6. Student organizations are limited to requesting two (2) tents and one (1) PA system at a time, unless previous approval has been given by the Office of Student Life. 
7. Coolers should be returned emptied and clean. Please allow them to dry before returning them to the Office of Student Life. 
8. All items can be picked up from the Office of Student Life one day prior to the event, and must be returned the following business day. 
9. It is the responsibility of the organization requesting the item(s) to secure them during their reservation and return the items in the condition in which they were received.  
10. All equipment is available on a first come, first served basis. 

By submitting this form I agree that I am responsible for the above item(s) during my usage. I understand that if the above requested equipment is not returned in the condition it was received, I will be responsible for purchasing a replacement.
