USC Upstate has vaccine requirements to protect the campus community from contagious illnesses, in accordance with U.S. Centers for Disease Control & Prevention’s (CDC) recommendations.
Proof of vaccinations
Proof of vaccinations are required for taking classes on campus at USC Upstate.
All Students:
- 2 doses of MMR vaccine
- 1 dose of Tdap
Students Living in Campus Housing:
- 1 dose of Meningococcal vaccine (MCV4) received after 16th birthday, if living on campus
Individuals vaccinated in South Carolina may download a copy of their immunization record on the DHEC SIMON Public Portal. You may submit these records, or other official vaccine records, at www.uscupstate.edu/myhealth.
Due Dates
Students are required to submit their immunization records NO LATER than 10 days prior to orientation or course registration. Failure to comply will result in the inability to change, drop or register for future classes.

How to get vaccinated if you are missing a requirement
Most insurance plans cover these vaccinations. You may go to a local pharmacy, health department, or doctor’s office to get these vaccines. Many urgent care clinics also administer vaccines.
Students without insurance, or with Medicaid, who are under age 19 may qualify for the VFC program. Please call (864) 503-5191 to make an immunization appointment if this describes your situation.
Additional information
The State of South Carolina requires institutions of higher education to inform students or their parent (guardian), about the risk of contracting meningococcal disease and hepatitis B, as well as the availability of preventive vaccines. Because most cases of meningococcal disease are diagnosed in teens and young adults, ages 15-25, USC Upstate requires all students living in University housing to be immunized with the meningococcal vaccine, as listed above.
The hepatitis B vaccine is strongly recommended for freshmen entering institutions of higher education, if this vaccine has not been previously received. The State of South Carolina added the hepatitis B vaccine to its immunization requirements for admission to elementary, middle, and high schools in 2004. By preventing hepatitis B, an added benefit of this vaccine is a decrease in the incidence of liver cancer.
