All students are required to have a history of current immunizations on file prior to enrollment. All clients will complete a medical history questionnaire on their first visit to the office. There is a small fee for some services to cover the cost of vaccines and lab work. For more information and to access the required immunization form, please see Immunization Requirements.
All clients may request printed copies of their medical records for personal use. Medical records are provided free of charge to outside medical offices and facilities.

As a student at USC Upstate, you are eligible to purchase health insurance through Academic Health Plans. Only students in specific programs are required to purchase insurance or submit proof of coverage, though all students are encouraged to carry health insurance.
The Student Health Insurance Plan is administered through Academic Health Plans and is underwritten by Blue Cross Blue Shield of South Carolina, and is the only in-network plan at USC Upstate Health Services. Students without the student health insurance plan may incur charges for fee-for-service items.
Provide proof of other coverage to waive the AHP plan if insurance is required by your program.
On the AHP website you may chat with an agent or communicate via email. Call toll free to ask questions and speak with an AHP representative by phone at 1-855-844-3015.
